Health Related Behaviour Survey

 

HEALTH RELATED BEHAVIOUR SURVEY FOR YEAR 4 AND YEAR 6

 

Our school, along with many others, has agreed to participate in a very important Health Related Behaviour Survey of children and young people in the city. This survey supports the work of the school to improve the health and wellbeing of our pupils. It is being supported by Newcastle City Council, working with its partners.

 

The questions that Year 4 and Year 6 pupils will be asked concern a wide range of health behaviours, for example:

 

  • Relationships, emotional health and well-being,
  • Tobacco, alcohol, drugs
  • Internet safety
  • Healthy eating
  • Exercise
  • Dental health
  • Home and family

 

If you would like to see the survey questions, please ask at the school office.

 

Each questionnaire is anonymous and confidential, and no pupil will be identified when the data is returned to school. Your child use their home postcode on the survey, but no individual’s answers will be linked to your child’s name or address.

 

We hope that parents/carers will agree with us that this is a valuable survey. We will assume that parents/carers do approve of their child’s participation in the survey unless we hear otherwise from you.
